Sekai, Kotonoha, and Makoto can all be somewhat upstanding people. In the games, its all dependant on the player's decisions for Makoto. Just as Makoto, Sekai, and Kotonoha can become absolutely detestable.
Remember, their is no one way for the character development of these guys in the games.
Kotonoha has no control over Makoto and Sekai. There is no way she can even ensure they will still be right there by the time she gets upstairs to commit suicide, let alone ensure that her death will traumatize them enough that they will split.
The main thing was to quit life, anything else was incidental. If she really wanted to personally ruin Makoto and Sekai, she'd have not killed herself and instead dedicate her life to making them miserable.
Death as a couple breaker is too much of wild card. It could very well have the opposite effect and bring them closer since they might deal with the trauma better together rather then alone. And if that had happen, she wouldn't be able to do anything else because she's dead.

Yes, I also love this story with a variety of human touch! But I bet LxH is not worth to spend tens of dollars (except this episode "Blue Smile"). This PS2 port spoiled the content of original game due to restriction of violent and sexual expression. Even so, it is the only thing I can say with conviction, this is the sole valuable epilogue among the additional ones in PS2 version. The others leave bad aftertastes only. On the other hand, I learned through this scenario that irreparable mistake would bring us lifelong suffering and regret certainly.
This tragedy reminds me of Françoise Sagan's debut novel "Bonjour Tristesse".
Certainly Makoto and Sekai were not honest with kotonoha. However, who can expect such a consequence that Kotonoha would commit suicide? Both Makoto and Sekai seem to have been suffered from this bitter memory. Especially, Sekai looks like being cursed by Kotonoha.
