My understanding is that this torture never actaully happened, both the sack, and this version of the sadistic Asami are constructs of his mind. She is an allegory, the personification of his guilt about his wife, and that sack is the metaphoric composistion of his insecurities about her abuse when she was a child. Disturbingly beautiful film.

I'm pretty sure it happened. At the end she is reciting all the things she said to him as though she were reciting lines. She had been looking for a victim/lover the whole time. The comments about the "others not understanding", the comment about her being a heroine of her own life and not the heroine in a movie, and the recitation at the end are supposed to make you see that it was never her auditioning for a role for his wife, but he was auditioning for the role of her victim

I think that should have been handled differently. But she is no figment. As a side not, how pissed would we be at a son like that?
