The Deadly Reflex mod -- how do you increase the chance for slo-mo killing?

I've done it countless times in combat and I find it hilarious, but I get pissed at how rare it is.

One time I landed a stab at the guys heart when he opened up an attack, the sword went through and I decapped him. Sick shit.
