There's one in the headmaster's office inside the school, one in your dorm room and one in each faction hangout you can unlock. The book is always on a table and you just go and press triangle there.

Yes. Where the lady is behind the counter, there's a book on the counter (there's also a rubber band on the floor there) and you save using that book.

I never thought saving the game would be such a huge puzzle for someone.
