I've heard of crush injuries and deaths due to fork trucks backing into people and trapping them between a wall or column.
Solution, put a shut off pad over the entire rear portion of the truck. If something connects with the pad over a certain pressure, the truck shuts down. Of course you leave appropriate openings for vents/exhaust.
Or insist on work practices that means pedestrians and forks do not share the same space. You can make it happen if you really want it to.
@SauronsEye Indeed 49% of industrial accidents are from people being hit by vehicles so its a big issue. I see what you mean with the "bump bars/pads" you suggested but I think on such things like a FLT by time it detects an impact and cuts power/brakes yo will be crushed anyway. The main countermeasure for this is awareness... driver training and pedestrian rules such as not using conveyence routes, we have these rules at Toyota and we have a very good safety record.
