The reason he managed to know where the bullet was was because he listened to the guy's tone while counting. Notice that when he says 5 his tone goes up and when it wasn't in five, Derren takes a break to think it over and picks up that his tone was already up when he counted 1 which seemed harder to pick up.

If you listen carefully to the things Derren says from 3:15 till he starts the game, it's obvious that Derren is trying to make the person choose chamber number one. He's invading his unconciousness with sentences like "I want you to choose ONE of those numbers", "Look at them now and CHOOSE ONE" and "Are you thinking of ONE now?" He's probably been doing this from the moment they met eachother. ;)

first bullet is chosen by partner, it's the only true one in the set

second is chosen by the spectator, it's a fake bullet witch doesn't even make noise, it's just a piece of metal.

the gun has an hidden device wich fires a little charge,so it makes noise and smoke like a real shot(in the end u can't see if there actually is or not a new hole in the bag)

derren has just to guess wich was the number chosen by spectator, but he's sure that the magic wil look great from the outside anyway
