Back in those days when sword fighting was common, there were many reason to how you could lose but I only know 4.

1. If you're sword breaks.

2. If your sword is knocked out of your hand by your opponent.

3. If you get knocked on concious.

4. If you die.

1 and 3 happened to Kenpachi. Ichigo dropped but he didnt get knocked on concious, check the rest of that episode if you dont believe me.
