sorry but crawling has become a growing problem in the game. decision was harsh, but necessary. Adam AC has gotten away with worse in the past. Schalk Burger is also another compulsive crawler. Once you're held on the grown, you can't propel yourself forwards - exactly what AC did here.

If it was given a try I would have no problem with it, most refs would give it. He was tackled 4 metres out and he must immediately release the ball. If it were forwards barging for the line from close range and the ball carrier was grounded but then driven over it would be a straight forward penalty. This is a similar situation but the fact that he is lying on the Tahs player makes it a little less clear cut but he is still tackled.
