I thought it was on one of the The Man Called U.N.C.L.E. episodes. I myself only got to see this episode just once when I was little when they showed in syndication, & it was right during when they recorded "One Less Bell to Answer" which had the scary ending added to that song.
I'm still looking out for that episode, but glad to see part of it. And "Puppet Man" is also one of my #1 favorites by The 5th Dimension.
