@skylarsartre it was havoc in the ER because of a massive accident and in all the confusion the residents sort of neglected a patient(a doctor forgot to do a crucial exam) and she died.The whole episode consists of flashbacks as all the involved doctors testify in front of the board,and each tells his/her version of what happened.
