It's most likely that they developed in utero as identical twins with 1 fertilized egg splitting into 2. However, unlike other identical twins the 2 fused together at some point in their development due to stem cells finding like stem cells on the other twin. Stem cells do this naturally, that's how different parts of the body form. It isn't anyone's fault.

On top of that, the fact that they have two separate hearts would mean that with one out of the picture, the other would have to immediately attempt to pump blood through the other half of the body.
