I'm not sure exactly how you'd lose a hand in that thing, it would possibly be mangled to the exent it needed to be amputated. And that would actually be even MORE painful than simply having it severed since it would be a slow death and you'd feel it in your entire hand.

If you've ever injured your hand or broken a bone it gives you an idea of how bad having your hand filed off would hurt. The man could have died from shock (or the blood loss). It's fictional but that doesn't make it much better since it was portrayed realistically. Just like the fact the rapes in "Deliverance" were fictional doesn't help much.

Most men wouldn't be laying still just grimmacing and screaming, IRL the pain would be bad enough that your body would be squirming involuntarily and you'd be jerking your arm around wildly. On a pain scale of 0 to 10 the pain would be around a 10 or 11.
