According to mike in an AP magazine interview, he lost it when he was a kid. he was riding a bicycle under a net and got his tooth caught on the net and it pulled it out. then over the years it has been reattached and knocked out several times. this time it fell out cause he head banged into a bass drum mic during warped tour. but now its back in. so he has all his teeth again.
