Ive translated the first 3 lines..

Tere paas aane ko dil chaahata hai- My heart desires to come close to you

Mohabbat ki ye manzille dil ki raahe- The destination of love is the paths of the heart

Its basicly about how the heart want to go somewhere far away, and unite with its love. And the later stanzas say that the love has changed thier lives..and how deep is thier love.
