1. Well, this is about a forbidden love of a Geisha-girl. Geishas always take a ricksha to the working places, which are called "Ryotei", a kind of luxurious restaurants. She askes a Kurumaya-san whom she knows well to take a love letter to the man she loves.
The Kurumaya-san asks his name. She replies:"fool, you know him!". The Kurumaya-san takes the letter to him.
The song is about a woman that asks to a rickshawman to deliver a letter secretly and to come back with the reply, always in secrecy.
Anyway she doesn't tell him the name of the person to whom the letter is addressed (I don't know why). In the second part of the song (cut in this video), the rickshawman come back without the reply. He actually opened and read the letter.
I think the name of the addressee is written on the letter. So when she changes voice she's actually doing the part of the rickshawman, who makes an unpleasant comment on the name of adressee once he has read it. Maybe he's jelous...
