Thank you for the great videos. I have fractures of pubic rami on BOTH sides. I feel as if I have some prolapse going on (very intense pressure with standing, walking and on toilet), although I can control urine flow. My ortho thinks I'm nuts. If this doesn't resolve, do I go to a gynecologist next or a urologist when the fractures heal?
@plagib You would want to see a doc who specializes in female pelvic medicine and reconstructive surgery--that might be a gyne with training in uro-gynecology or a urologist with training in female urology.
can a bladder prolapse be reverted or correct with the practice of these exercises? some people say than yes it can and others say that is a complete waste of time that exercising the pf muscles does not works! what is your opinion doctor?
@mariabonita423 Once the bladder has significantly lost its support mechanism, it is unlikely that exercise will reverse the problem; however, the exercises can help stabilize the problem. The PFM exercises are best for minor stress incontinence and minor prolapse.
great video. wish i had had u as my instructor in nursing school. i have never had any children but i have always had relaxed vaginal muscles. can this be hereditary. i remember my mom talking about this, and she only had one child.
Thank you for your reply ..I have also come across a website that sells Kegel divices to use to help this problem , do these devices work or do the exercises work better. Can you tell me what post-void dribbling means and will it get worse?
@pogonutaddict I have found devices to be gimmicky...try exercises and if unable to master, best bet is a good physical therapist who specializes in pelvic floor; biofeedback can also be useful. Post-void dribbling is leakage after voiding...you want to try to empty as best as you can, be patient, wipe up carefully, and if persists or worsens, see a urologist as it can sometimes be on basis of problem such as urethral diverticulum.
Hello doctor this really helped me a lot . I am a 57 year old female going through menopause. I have had 5 children . I was told i have Meniere's Disease ..i was put on diuretics on and off for the last 14 years mostly the last 2years .. I have been experiencing a little incontinence .. i experience this after urinating and standing up that is the only time .. Do you think it's because of the diuretics and will the Kegel exercises help this .
@pogonutaddict Sounds like post-void dribbling. Diuretics never help, although they are not the primary issue. Make sure you take the time to empty as completely as possible, and regular practice of pelvic exercises can certainly help.
hello doctor, im 31 yrs old and at the age of 17 i had a really bad motor bike accident and i was unable to pass urine and they insert tube and later i was ok, but now i think that i have premature ejac, will this exercise help me and when i tried to do this i can contract the muscles but cant hold it, the muscle gets relaxed, and i am under weight only 54kg with 172 cm height, so is this pre matu ejac caused due to accident or my under weight and will this exercise really help me
@Rockyyydude NOT the butt muscles (gluteals); NOT the abdominal muscles (rectus and obliques); NOT the muscles that pull the legs together (adductors). Feel your pubic bone with one hand and your tailbone with the other...the pelvic muscles are the hammock-like muscles that are between your two hands. Can stop urinary flow in mid-stream to assure that you are contracting PFM appropriately.
Hello, i was just diagnosed with a weak anal sphincter, trying not to be to gross but, i have a bit of incontinence from both my anus and penis. I'm only 25 years old, not overwieght at all, but have been pretty lazy the past year "sitting alot". Will doing these excercises fix my problems completely? I live in a smal town and the docs here just saying i'm being to "anal" about it, exact words. But i know there is more to my problem and was hopeing that this could solve the issues. Please Help
@beatdank626 If you are only 25 years old and are having urinary and fecal incontinence, then there is likely some underlying neurological or neuro-muscular basis for this. You really need to see a neurologist (nerve specialist) for further evaluation of this problem. The pelvic floor exercises may help, but it is important to get the underlying cause sorted out.
@oyu8 Flow is dependent upon numerous factors... bladder muscle contractility, urethral resistance, degree of filling of the bladder. These exercises will not improve the ability of the bladder to contract (squeeze) but will make you aware and strengthen the muscles that can stop your stream. At times, weaker flow can be on the basis of failure to completely relax the PFMs, and if this plays into your stream dynamics, the exercises can help facilitate relaxation and hence improved flow.
A component of PFMs are the perineal muscles...bulbo-cavernosus, ischio-cavernosus,etc. These can help compress the egress of blood from the penis, contributing to erectile rigidity. So they can aid with erections as well as ejaculation. Remember that erections are all about blood flow...so healthy eating, maintenance of a good weight, exercise, avoidance of smoking, etc, are fundamentally important.
I assume by disconnects, you are speaking about a cystocele...aka a dropped bladder. It generally does not cause pain, just a bulge and urinary symptoms. Kegel exercises may help strengthen the muscles around the bladder and weight training or anything causing straining is not a good idea.
If the bladder disconnects, as one of your videos shows, does that hurt? (I've had on & off lower abdomen pain, especially in the lower right quadrant for several years, but Drs and 5 ultrasounds find nothing.) Also, doing the keigels can't repair the detachment, right?
helo doc, there's still one questions which bothers me. Can you tell me the time someone with a strong pelvic floor muscle can hold it. And how long he can do those contractions. It would be very helpful, because than you actually have a goal.
@magily2 I advise trying to work your way up to a 5 second contraction...this will not be easy at first and will definitely require time to adapt. I would work my way up to a set of 25 of these, and try to do this three times daily. Before you know it, your pelvic floor will be toned and powerful.
@incontinencedoc you said if you can stop the flow of pee then you have control of the muscles. i can do this. but still during sex i dont last very long like literally a few thrust in and im already about to ejaculate. i used to last a very long time during sex. but i had a period of time where i wasnt having sex for like a month or two. then i met my current girlfriend and we have sex and im having this issue ejaculating early. im tired of hearing "already?"i hide it sometimes its embarrassing
Nice video. Sorry Doc, could not catch exaclty what exercises I should do ( male). Should I squeeze those muscle; is it the main exercise? Thanks in advance.
I think i'v urinary incontinance...going about twice a night and a good bit in day time too.. I seem to dispel the liquid VERY quickly after glass wter. I try HARD to put action the exercises before urinating but it seems i have to pee after some fatigue sets in! i'm 'hoping' and praying this tuturial will help me! i've only started the exercise programme about a week ago. Should i still be getting up..(out of bed) and peein? Shd i hav patients? Bless you
By all means, pregnancy is a great time to get familiar with, strengthen and tone one's pelvic floor muscles. They can be done any time during pregnancy. Any woman who is pregnant should always check with her obstetrician regarding the specifics of her situation.
No, you can do them daily--they are not high impact and they are not that demanding in terms of effort. You may notice some soreness initially, but that should rapidly abate.
tow question though: is there such a thing as to much exercises? you said 25 repeats, 3 times a day. can i do it more than 3 times?
one more question, about finding the muscles. if i see my penis moving slightly while making a short contraction, am i contracting the wright muscles?
Everything in moderation...but you can do more than 3 sets of 25. Should be able to see the base of the penis drawing in, and if you touch your perineum--between scrotal sac and anus--you should feel contraction.
Thank you! I have a practical exam today on the pelvic floor muscles and reproductive systems of the male and female today. These two videos were very helpful!
Your statement, "an excellent means of mitigating the incontinent is simply to do a rhythmic pelvic floor muscle contraction before or at the time of exposure...", has been of great help... Thank you Dr. Siegel.
CPPS is a huge subject, and there is no way that I can do justice to the subject in a short commentary. There are some excellent resources on Google--search CPPS.
Sir, could you provide some information about chronic pelvic pain syndrome? And what could be done against it? I have had super bad prostatitis and healed it with the broccoli therapy (Dr. Saracoglu). But the pelvic pain thing is still here. thanks.
I have had prostate surgery and have had incontenience for the past seven ys. My Urologist advised me to practice the exercises but seemed so uncommital about getting results that I never really tried them. I have been exercising for about three weeks and have seem some improvement. My hope is that I will see more improrvement as I continue over the next few months. Thanks for being so clear and most of all for helping me have some confidence that the exercises could make a difference.
Keep on with the pelvic exercises--they can really improve the situation, but may take a few months before the maximum benefit is realized. Be sure to pulse the muscles right before any situation that provokes the incontinence. Check out my youtube on post-prostatectomy incontinence for more info.
Wonderful video! For 15 years I've had on/off problems with very weak pelvic muscles resulting in swelling and pain when walking/sitting. This occurs when I stress my lower body with pushing or pulling heavy items or with some types of exercise. Your video reveals how important it is to keep doing the pelvic floor exercises because they really do help!
Thank you for sharing your knowledge. I have had two surgery and still experience stress incontinence. This information has been very helpful for me to visualise and put a program in place for myself. I particular took note about the triggers and time frames when to do the muscle contraction. That was particular helpful, not to mention the whole video. Many thanks.
... Wow. I finally know which muscles I'm supposed to be focusing on. Thank you so much for the imagery that you used and for putting this online. I've always read that you have to contract the muscle that stops urine flow, but I've never been able to really decide exactly which muscle it was. I've always been confused and wondered if I was doing it right. With this, I'll know that I'm doing it correctly, and won't stress out that I might be making my problems worse cause I was exercising wrong.

I guess the exercise to strengthen the pelvic floor muscles is exactly like when we hold or try to stop our urine flow and maintain that upward contraction for 5 second intervals and then relax. Will strengthening the pelvic floor muscles help with slight urine "dribble?" I assume so?
Yes, your description of a pelvic muscle contraction is very good and accurate. Once you have achieved better strength and tone of this group of muscles, you should likely notice improvement in urinary control--whether it is stress incontinence, urgency incontinence, or post-void dribble.
