Your upper guide is broken. You'll need to take the valve cover off and remove it. Nissan no longer requires the part. I had the same thing happen to may car. While you have the cover off you should check to make sure the tensioner is still good. it may be warring out which caused your timing belt to be loose and broke your upper guide.
My engine is shaking more than that... I'm still working on that issue.

use some carb cleaner, break cleaner or rubbing alchohol and clean your MAF then see if its still doing it. then check your spark plugs.
