It never ceases to amaze me how fools get put in charge of projects or expensive equipment, as a driver it is the MOST BASIC fundamental NOT to put a crane so close to the edge of ANY embankment especially on made-up ground, then rather than lower the load immediately/ASAP regardless of damage to the load he fanny’s around wasting time, then slews the counterweight which helps whip the whole thing over, you should always be at least the height of the embankment away from the embankment edge.
At the beginning of the video you can see a disturbing crack in the asphalt. The camera seemed to focus on it. I agree good job starting to swing the boom around but it''s yanked pretty hard at the end. Glad crane operator came through.
Operator did great at the end. I am guessing he was buckled in but he spun that cab around perfectly and probably saved himself a bit of injury or worse. Good job besides the whole unstable ground incident.
I'm not expert. However i wonder if they had a bigger crane with longer boom, it means they could be operating from a safer distance from the river and the ground naturally would be firmer and tough to withstand that weight
I run A.T.s in the U.S. and this would have been operator error, the boss can tell you what ever but it falls to the operator to make the decision. A 1 and a half to 1 ratio from the edge of the drop off would have saved him, if the hole is 10 foot deep you should be 15 feet away.

At 0:16 a crack was developing from left to right along the bank at the point where the front wheel was, the guy was even looking at it. Surely he would be monitoring it, but by 0:59 it was a good six inches wide, so it was clear that the whole edge was sliding long before that??

im a certified crane operator and the first thing he should have known was the stability of the ground {the VERY first thing} he is setting up on especially on a river because the stability can deteriorate very rapidly..which is what happened..plus swinging over an out-rigger puts all the weight on that one outrigger..operator is responsible..should have denied the lift..i have.better safe then sorry

The operator could have said no before he lifted, and it is his responsibility. In addition, it was not the operator who turned the crane, it was the counterweight which forced the crane to turn. One can not blame others for the operator chooses to park his crane as close to a riverbank. Therefore, the fault easy to Place the ... 40 inches from the joystick.
@trodat5200 - Doubtful the placement of the crane was chosen by the operator. That would have been predetermined as part of an overall plan by the project managers.

@trodat5200 I have been driving mobiles for over 30 years and i half agree...but these days in the uk hse is so tight ...ground loading is no longer the crane drivers responsibility (I override this when I feel the necessity...who's gonna argue lets face it )...but back in the day I always had a back up strategy....In this situation I would have put the slew pin in and shortened my radius rapid and dropped that bridge section in the river.....this would have been my plan before I started

they built up a shonky pad with subgrade fill minimal compaction up against a bank.looks like recent rain on rain has washed all the fines away through the fill.looks like another case of cost cutting that got you in the shit more.

Whoever did the grading on the lift pad really screwed the crane operator and his machine big time. Did anyone settle up with the crew that did the grading. They just backed up the dump truck and off loaded the wrong kind of fill and not even compacting it . Did the operator make it out alive?
It has nothing to do with the LMI. The load was next to the truck. The crane was as strong as it can get for that size crane. The ground gave way. The crane was to close to the soft edge!!!! Somebody should have researched the situation a lot more.

I cant understand why the crane wasn't abandoned as soon as the cracks appeared in the ground. If I were the driver I would of been out of there the moment it was spotted.

Accidents happen.
A few years ago, my boss flipped the boom on his big Lorain conventional because of a sticky clutch at high boom angles.
Not really the fault of the NZ operator, because he was probably just doing his job. The Super on the ground is the one who makes the call.
Expensive lesson...regardless.

The operator could have said no before he lifted, and it is his responsibility. In addition, it was not the operator who turned the crane, it was the counterweight which forced the crane to turn. One can not blame others for the operator chooses to park his crane as close to a riverbank. Therefore, the fault easy to Place the ... 40 inches from the joystick.

@trodat5200 - Doubtful the placement of the crane was chosen by the operator. That would have been predetermined as part of an overall plan by the project managers.

@GeoffBlackmore - Regardless of where or how, it's the crane operator's responsibility that the job gets done safely. You can never put the blame to others, regardless of what he has told from the management. A crane operator can and must always speak out if it is not safe to lift. As a crane operator, I could see from the beginning what would happen. He should not have started lifting.

@trodat5200 I have been driving mobiles for over 30 years and i half agree...but these days in the uk hse is so tight ...ground loading is no longer the crane drivers responsibility (I override this when I feel the necessity...who's gonna argue lets face it )...but back in the day I always had a back up strategy....In this situation I would have put the slew pin in and shortened my radius rapid and dropped that bridge section in the river.....this would have been my plan before I started

@0salot If your comment is correct, ALL employees need to be empowered to refuse to perform an activity that they deem UNSAFE or have doubts about.
Just because he's the foreman doesn't give him the authority to OK a lift that will end with the crane in the river.
Unless the contract calls for the crane to end up in the river.
Not to mention the operator is the one that's hurt....NOT the foreman.
Pretty silly for the operator to lift if he had doubts.

blatantly obvious if he maxed the lmi when he was jibbed up that far the other crane would have gone but it didnt he was letting the rope down but the ground was giving way quicker than it would come down this is because of the amount of ropes on his jib i reckon he had four fours on because of the wait load this equalls a slow up down motion on the rope this is why he couldnt deck it in time and yea the ground gave way its his quick reaction at letting the rope down that saved the other crane

they built up a shonky pad with subgrade fill minimal compaction up against a bank.looks like recent rain on rain has washed all the fines away through the fill.looks like another case of cost cutting that got you in the shit more.

Whoever did the grading on the lift pad really screwed the crane operator and his machine big time. Did anyone settle up with the crew that did the grading. They just backed up the dump truck and off loaded the wrong kind of fill and not even compacting it . Did the operator make it out alive?

It has nothing to do with the LMI. The load was next to the truck. The crane was as strong as it can get for that size crane. The ground gave way. The crane was to close to the soft edge!!!! Somebody should have researched the situation a lot more.

@no1wrench Stick to the wrenches bud,its very clear the ground gave out under the crane.The only thing that was "bypassed" was the money for a proper pad to suit the machine.

Im not familliar with the load chart... but the opperetor was anything but smooth with the load.... Im no opperator but asa crane mechanic for 11 years I have seen this too many times.... shoulda brought a bigger rig and NOT bypassed the L.M.I.

I cant understand why the crane wasn't abandoned as soon as the cracks appeared in the ground. If I were the driver I would of been out of there the moment it was spotted.
