That is a Casablanca Delta. REALLY good fan you got there! If the flywheel was broken I guess it would not run at all so like it can start on high but makes that sound, the flywheel on it must be just loose and fallen and the reason why its making that sound must be because the flywheel and bearings is sitting on the switch housing and rubbing against it and also the weight of the blades against the switch housing makes it too heavy to start on low and medium. Do what thfanman told you to do.
of first off that is a casablanca zephyr ceiling fan you are going have to take the blades and light kit off the fan ok then u might need to remove the motor housing
then the part that the blades were attached on the motor shaft it is a rubber piece slide it up about a tenth of an inch then will see an alen screw on the rubber flywheel tighten that screw so that the flywheel does not rub on the swichhousing this is a good fan made in usa its just the flywheel came lose
Replace this fan and buy a new one.

@5876mg No. It's too good of a fan to just waste like that. It is a fixable problem. All it needs is a good cleaning and replacement bearings.

You need to oil it. Depending on how it's built, it may be easier to lube it if you dismantle it from the ceiling first

Yes, indeed a broken flywheel. Been there. It's fairly simple to replace if you know what you're doing.
