Lateral knees can sometimes be tricky, but when looking at the films themselves I usually look to see if the head of the fibula is just behind the proximal tibia, and can tell if there is rotation.
I also look for the adductor tubercle on the medial condyle, and then make a judgement on rotation as well.
How I do my lateral knees, two fingers underneath the patella.. almost perfect everytime
