I never paid attention to that, I always thought the guy said, "Cafe Americana", which is a coffee drink, and thus the order would have made sense. But now I am left perplexed, once again, at Tommy Wiseau's screenplay. I guess that is why he tells us to watch the movie a couple of times. It can be confusing, and you will always find new intricacies interwoven into the script as you re-watch the film.

Johnny specifying he wanted a medium was very important. He needed to establish the fact that he would no longer be satisfied with any less or more than he deserves. It spoke to his character's growth and current place in the tapestry that is "The Room".

Also, the cheesecake represented loves enduring promise.
