@tbarnes6794 Hi sorry for the late response. Yes I do think you can get 20 mpg in an 03 f150 supercrew unsure about the tire though. They have a major impact on your MPGs I just got a new set of tires same size as my old ones but different brand and more aggressive tred pattern and my mpgs have dropped about 2 mpgs . I also have a 2 inch lift so no worries there. I would start with a cold air intake system and a Electric fan kit for starters . Change to any synthetic oil.
@JBPHOTOGRAPHY76 Thanks for the feedback and the video. The tires are BFG All Terrains btw. It seems like ford wouldn't have put that manual fan on the engine ya know because it hurts the trucks performance... oh well. Oh by the way, you might want to check your truck's oil stick tube. They can sometimes rust in half where it is mounted onto the side of the engine block.... mine did that if you were wondering.
@mr7confused Thanks by next summer I am going to be doing an edelbrock throttle body , JBA stainless headers and a magnaflow 15609 catback exhaust to really get the air flowing smoothly. I dont expect to get more than an extra 1.5 mpg at best but it will be about a 50 horsepower gain. So I will have around 330 horsepower and getting 26 mpg highway with a full size truck.
I do want to note that after to switching to AMSOIL that I did not see any increase in fuel economy and this was using their Signature series oil and transmission fluid.
@cumminsfever09 The edge was $439 from amazon you can occasionally find them for $399 but good luck. Also check to make sure that whatever programer you buy work with your particular truck and engine. If you do have a cummins diesel this programer will not work your truck but others are made specificaly for diesel engines.

I've got a 03 f-150 supercrew 4.6l. Do you think that I should be able to get around 20 to 22mpg? It has 35" tires and a 2" lift though.

1) magnaflow (being a flowthrough muffler) would be your best option for muffler in terms of mileage
2) upgrade your battery ground wires as well might have some good affect on your mileage, freeing up some electrical power for your fans and engine.
Great job though, I'm amazed that you could milk 25 out of that truck. My dad's f250/5.4 gets roughly 11 or so...its horrible
