I think the game is actually programmed to make that happen. It just detects a large flying piece of wreckage and spawns you at a point where it will meet you. Especially if it was your own wreckage.

My friend had a fail just like this in forge.

He was standing on a platform up in the skies of Hemmorage with a rocket launcher, and I was standing on the ground below, but not under the platform, And I was away from game.

He launched a rocket launcher at me, then moved back and accidently puled the trigger a little to late, and fired a rocket at the floor and blew himself up.

He spawned on the ground next to me, and the first rocket hit him about 2 seconds after he spawned and killed him XD.
