I earned it via the 70-hour method. It's doable in 70 hours without dustia, maybe 60 with dustia. My guide on GameFAQs explains it. I'm also working on remaking the video series on how to get level 99 fast & early - it uses an inefficient method. The new method is already much faster - level 35 in 10 1/4 hours after clearing Nalbina Dungeons, 13 levels (from 22 to 35) in only 2 1/2 hours. This remake will also cover the 70-hour method and getting the game's near-best equipment too.
While I don't have a video that really explains it directly, the guide is on GameFAQs - see "powerleveling guide". There are still a few things that need to be ironed out in that guide, but I'll get at that once I finish making the video - it won't be until quite a bit into September by the time I finish, if I actually do finish.

The 70-hour method is definitely the best, but it comes with a very troubling downside - the extreme repetition required is enough to steer most anyone away early. 2 1/2 hours to get level 99 from starting 98 is no joke.... I am remaking the video detailing this process (the one I currently have is highly inefficient and quite out of date). I'm level 66 right now with still under 20 hours on the game clock, for comparison.
I've recently got a case of level 99 being done in 81 hours. Perhaps 75 may be possible. At least, that way, everyone will be level 99 when you first get them. However, the 50-hour method is better mostly because 4 level 99 characters is more than enough for nearly everything, except Yiazmut. For the other two, the Cerobi Steppe (cast death on the flying enemies with full HP and the serenity license), or the 25 skeletons in the Nabreus Deadlands are the best ways to level the other 2 up.

This is because, early in the game due to the lack of good armor, the enemies can easily do 50 to 70 damage meaning if you're hit twice, it's game over. Just using a phoenix down is all you need to defeat dustia (and fidalient). I never saw dustia though, I use my own, less-risky, just-as-fast method.
Weak? It's the most powerful weapon available in the game at this point in the game's story. You won't find anything more powerful for the next half of the game's story! Why don't you try defeating a dive talon (in the Westersand) before doing the Nalbina Dungeons event. I can defeat them quite easily at level 18 where they can defeat my own character in just 2 hits. I even have a video showing how easy it is and it's part of how to get level 99 within 50 hours of game play.
do you ever initially play a game "normally" as in you level up with a more natural progression so the game is played at a more "developer-intended" difficulty?
I, essentially, don't play RPGs "normally". I've always been after extreme levels early on. This case is the most extreme I've ever done in FF12 and the next earliest point possible is that before defeating the Rogue Tomato and there's no worthy enemies for that outside the very dangerous "wild saurian" (this would have to be defeated using quickenings). This, however, can easily take 300 to 400 hours to do so I really don't see much point in doing it.

FF12 has a different battle system. Although it does have weak points, especially with ranged weapons (it seems like ranged weapons haven't been tested much), I like FF12's battle system more than any other FF game's system. FFT is the next best.

The automatic movement is the other reason - it gets annoying quite often. Imagine immobilizing a powerful enemy. You have a ranged weapon available so you can pound at the enemy safely from afar, but gambits can make you run to it and take a hit.

I was used to only 20 levels being present in a game, but 60 came and went, and it continued to 99. Even my book revolves around this kind of thing (though battles are the focus of my book).

I thought you meant 5 years from starting a new game to defeating Sephiroth. At the time, I wasn't into RPGs at all, almost completely unaware of their existence. I have seen them earlier than that though (FF6 is the first, from watching two friends struggle with it back in 1995 or so). I also played Ultima Exodus, but never understood it and didn't realize it was an RPG until more than a decade later. Final Fantasy Tactics was rented, but I found it way too hard (I never saw the world map).
Do you like the Shin Megami Tensei games, or have you ever tried them? I think they're much much better than most of the Final Fantasies, but I'm not sure if they'd appeal to you.
You really should try an SMT game. Maybe Digital Devil Saga first, since it's more linear like a FF game. You can also grind a lot in the games so I think you'd like them.
