Im pretty sure that the vast majority of muslims killed by the USA would fall into two categories...islamic extremist fighting us in iraq/afghanistan, or an unfortunate casaulty of war (i.e. collateral damage) but we do what we can to try to minimize and prevent killing innocent civilians, reguardless where we might be, weather it be the mid-east, south east asia, or wherever else we might be.

The reason why the ISAF forces haven't beaten the Taliban is because the Taliban keeps recruiting new members and the locals are too scared of them to truly help ISAF. The US couldn't easily wipe them out simply because the insurgents are hidden in the mountains where our guys can't find them.
