I believe (my intrepretation from way too much reading and Kurt collecting), is that he says "Knows not what it means" and follows with "When I say oooowwwwww", or somethign to that effect.

His stomach problems is what he is relating to here, saying his friend loves to sing his pretty songs, and sing along, but he cannot understand the pain and turmoil Kurt goes through with his stomach to achieve the musician life, touring and such with a messed up stomach.

@gussy0896 I think this is actually from the original recordings of nevermind that they did with Butch Vig for Sub Pop, before they signed with Geffen. They had to abort the project because Kurt blew his voice out on lithium and hadn't finished lyrics for all of the songs yet.
