i think i got it... the menu the waitress brought had the card stuck between the pages. (waitress knew which card because everyone saw it) and when he opened the menu and put it upside down on the table, the card was still stuck between som pages, saw the spectators couldn't see. obviously something had been planned and the card was between the pages under a certain word on the menu where he would have seen and stabbed right on it. after, he just pulls the menu away to hide where the card was.

The card is just placed in the menu by the waitress, notice that david doesn't flick through the menu to show that its not tampered with, its just between 2 of the pages and when he stabs through and rips the menu away, the card stays stuck through the knife

Waitress witnessed the card being shown, grabbed that card from fresh deck.

Card was either stuck to inside (underside) of menu before it was slapped down, or otherwise handed to him and quickly slipped under there, but attached in a certain position.

If waitress or owner wasn't in on it, the owner would have been pissed off about table damage and damage to a menu insert.

How I think he does it:

Blaine makes him show everyone the card, So his helping friend (the waiter he gets the menu from) can see the card.

Blaines helping friend puts a duplicate card between 2 pages inside the menu.

Blaine holds the menu so the card doesn't fall out.

Blaine rips the menu out so no one can see the card is between the 2 pages.

Maybe he had a partner who saw the card then pulled a copy of it from a second (marked) deck that was kept hidden the whole time and threw it onto the pile when it was being mixed up. Then it's just a matter of spotting the marked card and aiming the knife at it.

Sleight of hand is right @ 1:54. Shuffler and guy in grey suit both are plants. Shuffler keeps track of his own card, cuts deck purposefully in half at that his card, slides right half of deck towards grey suit, grey suit lifts the card, card is placed under menu by plant waitress after hand off.

Look at the table prior to the menu being placed on it and then skip to where the card is stuck under the knife. As already mentioned, there is a pocket in the menu and the card is ripped out of said pocket and remains on the table. He obviously had an accomplice place the card in the menu, hence why he had the card shown to "everyone". I really like card tricks but they have to be sleight of hand. I can't get into this accomplice stuff. Chris Angel is the worst...

But I picked one card up, and which was it? 8 of Diamonds. Not fucking kidding. But the difference is that I just was very lucky, and my trick took no skill. :)

That was easy - accomplice or cameraman saw which card the guy picked then told another accomplice either behind the scenes or via a microphone to place that card in the menu, then david blaine just stabs that area of the menu where the card was.
