Im German, so I can say that she sings the original german lyrics by Heinrich Heine but with an obvious accent. It seems like she got told how to pronounce which words, without understanding it.... but its ok ;)

It was in Fate/Stay Night. They play it only when there is a sentimental moment with Ilya and Shiro, and in Ilya's death in Unlimited Blade Works when it goes on about her and Berserker when they were still in Germany.

Yeah, Ilya is German, so she actually sings it during one of the arcs as you walk her home. Well, it's not voiced, but the BGM is Die Lorelei, and the lyrics are there.
