This was at the molson amphitheatre. Its got to be one of the best places to enjoy a concert. Its not huge so moving is relatively easy, and theres a good 2 feet for your knees not to hurt in.

In interviews and whatnot it's always been said the same: A friend of Cash suggested the song to him, he loved it and they asked Reznor for permission. Look for the interviews and declarations of Reznor and Cash.
Also, in the official presentation of the "Hurt / Personal Jesus" single credits fully went to Trent Reznor, some google will show.

But, yeah SEVERAL people have operated LONG under the assumption that Cash did indeed "Do time in Folsom", and this theory goes all the way back to the "Folsom Prison Blues" single release. The theory was propagated even further when Cash recorded "Live at Folsom Prison"; the general consensus among the otherwise intelligent, but uninformed masses being that the album was recorded while Cash was serving his sentence. That's not to say that Cash was never in jail, however...

@wyattscurlock The version of the song that really hit it big though was actually the NIN remix. It's even on one of the "Best of Bowie" cds. That's why you can hear Trent singing in the background of the "god is an American" part.
