I wound up replacing the injection pump with a 4911 from a '93 6.5L turbo. It was pretty much a direct replacement aside from me having to turn down the fuel output. If left stock it would have overfueled the engine resulting in high EGTs. To sum up, the old IP was bad.
I put a new IP in it and adjusted the timing until it had a good idle, good diesel rattle, and no smoke. It runs like a dream now.

it shake and run like crap because you retarded the injection.
you have to advance it a bit : it must be rotated towards the passenger side.
cold start system advance the timing.
keep the marks on pump and block aligned then rotate just a hair passenger side.

You should watch the other video I did, after the new injection pump and with the straight pipes. It sounds a bit more aggressive.
