The song is about a guy who loves a girl but keeps causing them to break (he is the modern leper) but the girl keeps coming back to him, he knows he's the problem but he can't fix it no matter how hard he tries, and is always conscerned that this time she's not coming back.

The lyrics are pretty open to interpretation, but I believe it is about a guy who believes himself to be a failure and nothing more. But while he wallows in his despair, he has a person he loves that, to his confusion, keeps coming back to him even after he continuously hurts her emotionally. While her repeated visits keep him from going off the deep end, they also serve to remind him of how much of an ass he is for hurting her. The end can be either him getting better or just a repeating cycle
