I did this a bit differently... made a volumetric hologram encoding to give inline, achromatic, multi-focal reconstruction. So, lathing a refractive profile?... not necessary.
Ha! I used to do this for a living in the late 70's when I was in my early 20's. It's easy once you get used to is an have broken a few diamond cutters off the lathe.
