@alexshaun98 Exactly what do you think is there to aspirate with shoulder impingement? I've had cortisone and Lidocaine (not Lignocain) injections (a few actually) for this same problem (impingement / partial tear) and there was never anything aspirated before the injection(s).
@Soulrider2012@drmorb@alexshaun98 The point of aspirating is not to resolve an effusion. Before injecting the steroid, the doctor should pull back on the plunger and observe whether there is a flash. If blood enters the syringe, then the needle is in a vein, and the doctor should start the procedure over from step one, not risk injecting directly into the bloodstream.
