It's not as dumb as it looks. That he's using a drill makes it look really creepy, sure, but he was being careful, not just drilling away. Sooner or later the black area of the nail would get spongy and a hole would open anyway. This way the pressure was relieved sooner. I've had it happen but used a large needle to open the damn thing. As long as you keep it clean you're fine. I see a similar comment not far down from mine.

They do the same thing in a doctors surgery, but will quite often use a red hot needle. It lets the blood out that's building up behind the nail. If you don't do it, you'll lose your nail, and that can be way worse than a little hole in the nail.

Another alternative to a drill bit is to take a paper clip a small candle. take the paper clip and unfold till it is perfectly straight then light the candle heat up the paper clip end until red hot and press on to thumb. It takes a few times and smells like shit but it does the exact same thing only by doing this your finger nail will not heal back up and fall off the hole will stay unchanged and grow off with the nail!

actually I have done this myself except i used a hot needle to melt through the nail. It makes a finger with a blood blister under the nail feel alot better as it relieves the pressure which causes most of the pain. i thought it would hurt like mad too but you can't actually feel a thing
