The female has a large wound on her back that is white rimmed... clearly rotting. She is completely septicemic, probably delirious with fever and so weak standing is not easy for her, even without a lion on her backside.
The mother hippo was clearly very ill, Ive seen mother hippos in action before. If her calf was in danger, a few lions would have been dead. But I'm kinda shocked the calf put up that much of a fight.

A healthy hippo would march into the river with three lions on her back with little problem. The lions are filling their role of removing sick and aged animals from the eco system.
