Actually, the "rare side-step/trips-on-carpet" opening has Rob first BUMPING INTO the ottoman and THEN side-steps around it with some "pain" in his shin. There's another "rare" opening, but when originally used on CBS wasn't all that rare. It is the opening where the title says "The Dick van Dyke Daytime Show", used on the Mon-Fri daytime reruns on CBS-TV from 1964-69, 11:30am Eastern, 10:30am Central & Pacific. There's no announcer and the names of all the stars are flashed on the screen.
@MarkJ1961 And the visuals of the "D-v-D Daytime Show" opening are the usual Rob entering the house wanting to kiss Laura, but she points to Buddy & Sally. I don't remember if Rob trips, bumps-into, or side-steps around the ottoman though. This "D-v-D Daytime Show" opening has been seen in SOME episodes during syndicated reruns including Nick-at-Nite and TV-Land reruns in the 1990s.
You always knew when it was going to be 3rd opening with the stumble because Laura was wearing a different dress. I used to watch reruns of this so much, I could tell the other two apart immediately, too, because the gestures were slightly different.
The rare "sidestep and stumble" opening is from the classic "That's my Boy" flashback episode where Rob thinks they brought home the wrong baby (Ritchie)
Also -- to tell the "dodge" from the "fall" openings, look at Ritchie. His hand goes up to his face on the "fall", but stays at his side in the "dodge".
The "photos" opening was used for the first season. It uses a different arrangement of the music, so there's no way to synchronize it with the later openings.
