in the manga, Tar-chan got shot by a some rifles, and he was licking his wounds like an animal would. His student (the karate guy Pietro) noticed he had a gunshot wound at his back behind his armpits, hence the very disturbing action as seen here.

Also, I believe in this game you can also actually a) jump glide using your foreskin and b) dodge bullets and projectiles by wobbling your body. (standing still? can't quite remember...)
