(and before that, she was a special education teacher in Arizona).  She doesn't get, and doesn't seek, a lot of media attention for her charity work. Are you familiar with the role of First Lady? It's not required, but greatly welcomed, that the First Lady does philanthropy both in the US and elsewhere. The role of First Lady gives the President's wife an excellent platform to contribute and champion causes. It's been done throughout history.
