The bookshelf itself is fake but when it hits the table it smashes the cups and shit to make the injury look severe. Look closely at the TV, you can see it's made of rubber or something to make it LOOK real.

Look at the books at the bottom of the bookshelf, they don't slide out or anything they stay put.

Now look at the boy when he supposedly gets hit, he steps back as the tablecloth "hit him" and dropped after hitting his back on the shelf.
