X-ray glasses make it so you can see the map and all items on a level. The door with swords is a challenge room that is only open to you if you have full health. There will be a chest to open which will drop some items. Then you have to fight 3 rounds of badies. Also a good hint is if you have extra bombs, you should bomb for the secret room, I've found that it's most often the room with the most rooms surrounding it, and if there are multiple, it'll be the one closest to the boss room.
