I believe that the one replay where I broke my spine had a fracture threshold of 80, all other replays fracture was disabled. Engage distance varied. No mods were used, at all. The end.

Always start relaxed, never start out by hitting C. Eventually if you're doing it right you'll have manipulated every joint and will do stuff like relaxing a knee to have your leg begin to stretch out before it's positioned correctly to extend at the right moment (such as when it is lined up to strike for a decap or other dismemberment).

This is what I do.

For multiplayer though hitting C at the start is fine/recommended.

Looking back though, if you watch this video and are wondering what I'm doing at this point in my Toribash playing, the majority of these moves are starting with me holding all, with a few exceptions. What I just said applies to what I typically do now.
