When you use the music box, it puts all of the undefeated Hammer Brothers/boomerang guys to sleep, allowing you to pass them without having to defeat them. It's like how the cloud lets you pass by an undesirable level.

I love how world 5 is broken up into two parts. On the ground and in the clouds. I hate thoses jumping blocks. B&N tomorrow I hope. 9,779. 5:40pm. Thoses annoying lava balls coming up from the lava. The scary ghost., and U lost a life there. How did U get the coin ship?
