John Kramer was killed in the last movie along with Amanda. Strahm came into the operation room with three dead bodies and in self defense killed Jeff. Hoffman is the one who will carry on John's work since he and Amanda are dead. It was revealed that the trap he was in never killed him and Rigg figures out that it was Hoffman who is John's successor when he sees him escape from the test. Hoffman leaves Rigg to die because he was shot by Eric before he died and he locks Strahm into the room.

The truth of the ending is that the events of Saw IV occur concurrently with the events of Saw III - that's the twist. While Jigsaw was testing Jeff and Amanda, Hoffman was testing Rigg. The autopsy from the beginning really occured after all of these events; even after the events of Saw V depending on whether or not Jigsaw and Hoffman planned Strahm's true death to be in the Glass Box Room.
