I almost always perform this low interscalene block. I check whether local anesthetic spreads around the bracheal plexus in the supraclavicular region during local anesthetic injection as well as after local anesthetic injection. I almost always get good local anesthetic spread. When I find that I won't get good local anesthetic injection during the procedure, I move the needle tip placement to get good local anethetic spread.
I fill insertng the neadly in direct vertical direction will be more safe as it will avoid pherenic noerve and a smaller path will be traversed by the needle.
I call this technique low interscalene block. Low interscalene block is thought of as the anterior approach of cervical paravertebral block, which can block all branches of the brachial plexus.
Two complaints. Shit guitar music. Approach from the posterior is safer.
