When you read the other volumes.... the Sexuality bit comes more up front for you to view. One character comes from a whore-house. So that answers the small amount of confusion you have experienced while seeing that shot of the young girl. Hope you read the rest, it's one of my favorites.
I've actually bought the first 6 vols. of this manga and read a further 6 more, so I can state how compelling the story is and how epic the scope of characters there are. I've still yet to finish it but it's easily one of my favorite manga.
