@tadentt This is not a plugin. This is a camera effect using a manual focus.
1) Be sure you have a shallow depth of field. You can do this by opening your aperture wide and increasing your shutter speed (to avoid overexposure). You can make your depth of field even more shallow by physically moving farther away from your subject and zooming in.
2) Focus your camera on an object behind / in front of your main subject.
3) Manually adjust your focus so that your main subject becomes focused.

This is all done with a program called "Rack Focus" such as the title suggests.
