I have always thought that the orbits of the planets were constantly changing little by little, and every so often, when one has shifted enough, it will suddenly change positions in a major way. Small planets, taking orbit around another, usually larger planet, would be the most common scenario. Actual collisions I think would be extremely rare among the local group, but bodies entering the SS will have very different momentum/dynamics, and are more likely to hit something.

Just a hypothesis.
