My vision prescription is listed below:

RX - OD = Sphere (-225), Cylinder (-050), Axis (170)

RX - OS = Sphere (-200), Cylinder (-025), Axis (025).

Am I a candidate for LASIK?

@smiri013 i don't think it matters what your presciption is, just as long as your eyes are in good health.

Generally the USA the 'start' age is 18. But other criteria apply in terms of prescription and eye health.
