We never see who enters, and then all of a sudden - she yells at him and says "there's never been any us". It's just like such a big space of time just vanished. The dialogue matches perfectly the circumstance that Dan comes to her apartment after getting her text and she confronts him about his novel. Blair was fantasizing about what would happen once he got to her apartment, rofl :p It was part of HER imagination!
