Some guy tried to come in from outside and pushes the wrong way too hard. It's also an auto-rotating door so the motor and gearing prevented it from turning the wrong way. The glass support arms in the middle failed and the now movable part folded on the guy exiting.

@gnamp if you guys look closely you can see that the door was caught on the edge and bent until the glass shattered
